Self-employed or Contract Labor must have last two years Tax Returns.

COPY OF PHONE: In Applicant's name. ALL PAGES, NO EXCEPTIONS

REFERENCES: Seven References along with Name, Address, City, State, Zip, Phone Number and Relationship.
(4 of the 7 References must be RELATIVES)

PROOF OF RESIDENTS: Copy of the Lease (or any recent utility bill / bank statement) showing physical address.

COPY OF DRIVER'S LICENSE: Must be current and valid.

COPY OF SOCIAL SECURITY CARD.

COPY OF AUTO INSURANCE: Full coverage required (On All Financed Vehicles).

EXPLANATION OF ANY CREDIT PROBLEMS.
